Patient Health Information and Privacy Policy

This policy outlines how Patient Health Information (PHI) will be utilized in this office, as well as the patient’s rights regarding those records. It is essential that you read and consent to this Privacy Policy before receiving services. 1. The patient understands and agrees to permit this office to use their PHI for treatment, payment, health care operations, and coordination of care. The patient also agrees to allow this office to submit requested PHI to the payor(s) designated by the patient for payment purposes. This office will restrict the release of all PHI to the minimum necessary to process payment, ensuring HIPAA compliance. 

2. Patients have the right to examine and obtain a copy of their health records at any time and may request corrections. Additionally, patients can inquire about the disclosures made and submit written requests for further restrictions on the use of their PHI; however, this office is not obligated to agree to those restrictions. 

3. The patient’s written consent will remain in effect for as long as they receive care at this office, regardless of time passage, unless the patient provides written notice to revoke their consent. Please note that a revocation of consent will not apply to any prior care or services rendered. 

4. This office is dedicated to protecting your PHI and fulfilling its HIPAA obligations. Staff members have been trained in patient record privacy, and a privacy official has been designated to enforce these procedures. 

5. Patients have the right to file a formal complaint with our privacy official regarding any suspected violations. Furthermore, this office reserves the right to refuse treatment if the patient does not accept the terms of this Privacy Policy.

SMS Terms and Conditions (Text Messaging) 

1. SMS Consent Communication: Information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
